Patient Requirements
Age
All applicants must be 18 years of age or older to apply
Identification
Must have a US Government issued identification card, succh as a driver’s license, passport, in order to verify your identity.
Debit Card
A US Bank issued debit card is required for a down payment. All of your payments can be made by a credit card or ACH payments within your patient portal.
